I never got an email after passing 100k and 125k, so emailed customer service. Got a reply the next day with this link: http://www.delta.com/medallionthreshold
I had to wait a week after selecting the 100k reward before using the same link again to select the 125k award.
Also, if you are not sure if your selection went through, try using the link. If you have already selected your award it will say so.
Is anyone having problems receiving their rewards? I enter my skymiles number and nothing happens. Reached the highest threshold and can't receive any of my reward options.

Yes, but I just passed the 100 and 125K last week. I called SMS today and the agent read back to me what he saw. Apparently, the levels are checked every two weeks and notices go out then. Since it has only been one week for me, I told him I would wait a few weeks for the e-mail and if none comes, I would call back.

Hopefully DL will adopt the NW version of this: Elite ExtraPerks. I passed 125K last week and got the email the day it happened. Then, you go to the web page and select your award. I chose the 35K bonus miles (15k at 100k and 20k at 125k) and they post instantaneously. I hope DL adopts the automation.

Just a quick update--I was able to select both awards in succession this morning. No email--and I don't think it is related to whether you need the email or not to select.
I did notice that when checked on my wifes acct (FO), or when I don't log in, the "claim reward" button is not present on the screen.
When logged in under my account, I do see that button. In the past, I clicked it and another series of windows opened. One prompted me to validate my SM#. In the past, I would enter, and it would reopen the same window with nothing happening.
This morning, I lowered the internet options settings from medium-high to medium and gave the process a try again. I did get the screen requesting me to input my billing address for the award. After completing that, I was given choices to select the 100K threshold award. Just after selecting that, the next screen gave me choices for the 125K award.

Are the miles rewards cumulative? I thought I read somewhere that they were not, but I can't find where I read that. Will you get 15k miles when you reach 100k MQMs and then 20K miles when you reach 125k MQMs (in addition to the 15k miles), etc?

Yes, they are cumulative, so you may earn 15K+20K+25K=60K RDMs by reaching 150K MQMs and therefore earning the ability to select all three awards.
I received the threshold bonus for the 125k level today. Although I never received an email reminding me to make my choice, I visited the link posted up thread on July 1st and was able to make a selection.
Oddly; however, I am almost at 225K now and have been unable to make selections for the 150K, 175K or 200K level. Visiting that link now results in a message that says I have registered for every threshold bonus for which I am eligible.
