Mine is mostly from huge rollovers which grows every year as I exceed 125K MQMs. Most of it came from two years of flying to KL every 5 weeks from the US East coast.

Current stats:
MQM: 30,466
MQS: 23
MQD: $2,562
I don't have anymore flights planned for 2015, but should hit the Amex waiver soon, so will get the 10,000 MQM for 15,466 roll over. I might actually hit GM next year - with rollover and planned trips, I should meet FO MQMs by mid-January, then I have two TATL trips planned for spring/summer, plus my usual conference travel and short BWI-RDU hops.

So nobody is posting those numbers in a single year; however, if you have those numbers you are consistently flying 200+ a year, year after year with no break.

I will end up at 375K this year, with roll over it is not too crazy to see 400, 500 numbers. For me I was flying about 250 a year, so after the first year I rolled 125 and started the year with 125, flew another 250 so then rolled 250, fly a 3rd year of 250 and end up with a 500 balance and a 375 rollover.

MQS: 17
MQD: 3,800+
Rollover: had 0 coming into 2015 (this will be my first time flying enough to achieve status)
2016 status: FO
5K of the MQM are from the Amex Skymiles Platinum signup bonus. I decided to get the card about 6 weeks ago when I wasn't sure how much flying I'd do in the last quarter of 2015 -- there was a chance I'd end in the ~22-24K MQM range, which would've sucked. It turned out that I flew enough that it didn't matter, but the MQD waiver and extra MQMs will be handy next year.
Right now I have 27,990 MQM and 2,935 MQD; I'll be hitting FO on my next flight on Friday.

Rollover: had 0 coming into 2015 (this will be my first time flying enough to achieve status)
