That's what owners of casas particulares can routinely arrange for their guests----the next place to stay, the taxi or shared taxi or bus ticket to get there, a guide, etc.
What the expensive group P-to-P tours (like Intrepid or Road Scholar) provide, IMO, are their established connections to people in the arts, farmers, etc, etc. to easily arrange visits and discussions; a casa host also might be able to arrange such visits locally, but probably only once you got to that casa (doubt most would make a commitment by email), subject to their knowing such a person, and then as a courtesy. Those relationships take time to develop; hard to do for a first-timer.
