I got approved for this card at the beginning of August in-branch. Unfortunately I applied right before the current Olympic promotion, so i got a mere 17.5k points after $2500 spend in 5 months. Kicking myself now...

This card has a lot of VERY lucrative unadvertised benefits...

This is a great card for the advertised benefits. Double points on gas/groceries OR airfare, whichever has the most spent during the billing cycle. Also double points on telephone and cable. They also run the occasional bonus and waive the annual fee with $24k spend/yr.
This is a very nice addition if you want to fly free but still earn the miles but it is capped at 2% return if you can find the right ticket. This card originated from NW Worldperks and back then I was approved for 2 cards at the same time.
One disadvantage is that they have a very active fraud system, you can have multiple cards on the same account but they'll block when they see too many charges from different cities. I use the card a lot less because of this.
So if I purchase 4 $200 tickets in one transaction would it be 80,000 points or 40,000 points? Also, are taxes added after and paid or figured into ticket prices?
Each ticket is priced separately, and taxes and fees are included in the price before calculating points required.
For hotels you can redeem 10k points for up to $149.99, after taxes and fees, through the travelocity engine (a slightly better deal than the discount offered through the Flex Points rewards shopping). I assume no hotel points on these reservations.

US Bank is very conservative. I was told by my banker that if I had 2 or more credit pulls in the last 6 months I would NOT be approved for a home equity or car loan. I don't take this as "gospel", it's just what one employee told me. Tread lightly.
I just got the US Bank Cash + Visa. If the Flex Perks card is like that one, very few people got instant approvals ( based on info posted on other boards). US Bank is pretty conservative, and doesn't like lots of INQ's.

United flights booked on travelocity earn mileage, so I think you're good to go since that's how you use FlexPoints.
Having said that, I haven't actually done it myself, so I can't answer your question for sure.
yes. all tickets earn regular miles, its just like a revenue ticket. taxes and fees are included. if a tickets on delta.com, for example, is 200 USD all in, then you would find the same flight on travelocity.com for 200 USD or 10.000 points all in.
