Costa Rica Entry Forms

I have been looking for sample copies of the Customs and Immigration Forms for Costa Rica. These are reportedly given on the aircraft prior to departure, but AA said they cannot give these out, and repeated messages left at Immigration in Costa Rica go unanswered. If anyone has a sample copy of these forms - or knows where to get them, please respond back in this thread. I'm looking for the forms on behald of an elderly client, and we want to pre-flight him with the forms so that there is no hassle when he arrives. Many thanks.

The customs form, in particular, is poorly worded and can be confusing to anybody, and don't get me started on how small the print is.

"forseen address" is the wording on that form that confuses me every time.
