You've obviously never travelled domestic in America, especially to/from the south. I've seen them run out of extenders on flights. Southwest even has a policy for such people "passengers of size."
And in American hotels where the shower is also the bath they have fat-person shower curtains. There is a bulge in the rail at the shower head end to accommodate the larger body.

Tommyrot. Curved shower curtains give everyone, slim and plump alike, more elbow room. This enables thorough scrubbing of the underarms and makes those around you happier.
Showers that are not located above the bath are of course better still. Given that no one in their right mind would want to sit and soak in a Holiday Inn Express or Hampton Inn bath tub, one can only hope that in time we'll see more proper showers.

Pretty amazing.
I have seen it on BA flights and sometimes it seems like an awkward moment when the CC have to ask if they want extension, it is for safety and should not be anything strange about that.
Location: Sometimes in an extremely unfashionable corner of SE London and sometimes in Stockholm.
Programs: BAEC Gold and various other cards, mostly comped from that

I'm on the fat side, but never had a problem on BA (BTW, I think the CW and F belts are bigger to accommodate you moving about a bit when asleep).
However, a Kingfisher ATR a few months ago brought me almost to the point of needing one.. Thankfully, I could just about get the buckle closed - an hour of bring barely able to breathe was worth it to put off that ignominious day.
