Last year I had a ticket that I cancelled and was told I could use it towards another ticket after paying the change fee. If I buy a refundable ticket using the voucher (for more than the amount of the old ticket), can I cancel the new, fully refundable ticket and get a new voucher (with an expiration date farther into the future)? I know that this works on some airlines but not others.

Ok this makes no sense. I am trying to apply it to a fully flexible biz class ticket. However, I'm being told that if I were to change anything on the new ticket (including dates), it would incur the change fee. Shouldn't it be just an exchange of the old ticket for the new one??

Yes it does. The original fare rules of the 'old ticket' applies to whatever you apply the new ticket to including change fees.
It is not an even exchange as the old ticket is non refundable and if you use that voucher toward the new ticket, it sometimes can become nonrefundable as well.
You basically cannot escape the conditions of the old ticket.
