1. Three hours in JNB will be plenty, delays on SAA domestic are not frequent and - if there are delays - not long.
2. Its not far, but due to construction at the airport it can be a bit of a mission. Should not take more than 15 - 20 minutes though.
I just returned from South Africa yesterday and over the course of the last two weeks flew 9 flights on SAA. All flights, including domestic, regional and international flights were on-time, and in most cases earlier. A three lay-over in JNB should provide you with plenty of time.
I was pleased to see how the construction at the JNB Airport was progressing along. A connection from the international arrivals area to the the domestic departure area is now only about 5-10 minutes walk, all inside the new terminal. You no longer have to walk outside along the sidewalk or transit the parking garage to get from one terminal to the next. It makes the transfer in JNB, much more pleasant and they have added several new shops and restaurants along the way.

The International to Domestic transfer has been like that for some time, but is the same true for the other direction (Domestic to Int.)? I have not done that for some time and reports (admittedly a few months ago) were that this is still a bit a construction zone with a walk through the car park thrown in. (not that this would make the three hours connection too tight, as per my earlier response to the OP).
Yes, the connection from Domestic to International is the same. You no longer need to transit through the car park. You go from the arrivals level of the domestic terminal up the escalator to the departure level and walk through the new terminal to the international terminal. It is about a 10 minute walk, at most. The international check-in area is still the same, but once you clear security and immigration the duty free shopping, restaurants and gate areas have been expanded. I was quite imprerssed with how much progress they have made with the new terminal.
