Do you earn 2 points per dollar (3 points if Gold/Platinum) for spa services when NOT staying at the hotel (in the same way that you do for restaurants)? Also, do spa services when not a guest earn 2 points per dollar on the SPG Amex?

I made the experience (Hong Kong) that you do not even get points when staying at the hotel. They treated it as a cash charge out (so as they start to do for hotel cars operated by a so called independent operator)

Regardless of stay or not, spa services are not eligible charges to earn points according to the T&C's.

5. EARNING STARPOINTS FROM HOTELS 5.3. “Eligible Charges” are charges incurred by you on your room account for Eligible Rates, food and beverage (excluding alcoholic beverage purchases where prohibited by law), direct dialed telephone and direct-dialed in-room fax, laundry/valet, and in-room movies and video games only. Eligible Charges do not include:(d) other fees paid such as parking, business center, high-speed internet access, retail stores, spa products and services and greens fees
