This was the flight that glided into LHR. Turns out it did not run out of fuel after all.

Interesting read. They basically say that they are pretty sure there was ice in the fuel system at some point that caused fuel flow to be diminished at the end of the flight when more thrust was needed, but they cannot come up with an explanation for how such a volume of ice was formed nor how it got into a position in the systems to block both engines.
In other words, they have no idea. Or at least very little.
