Cathay Pacific Asia Miles - AM redemption on cx.com

one of the benifits of redeeming online is ticketing immediately so you can travel "straight away",
but have anyone experienced a mistake while ticketing redemption tickets online?
twice, both when redeeming for a nominated 'friend'.
1. Selected the nominated friend, ticketed, then realized the Title was wrong. my gf was titled Mr. I was 90% sure I've checked it right after I've selected her name so i called AM. It was then sorted out with a note on the eticket. A mildly akward face when she saw her BP but she got on the flight.
2. I travelled on my dad's points. Since I've had the prev. experience, I DID do it carefully. I SELECTED my name, I've seen my AM number and proceed. BAM. ticketed to my DAD! Since it was a last minute trip I went to the airport and hope for the best. Of course my name could not be found while checkin in, but it was quickily solved on the AM/MP helpdesk with a 3000 AM trade for a name change.
I'm wondering if this had happened to anyone else or it was simply me being careless? and is there something to be done with the 3000 AM which i think i needed not to pay for?

There were also times when everything went right when i did the same, I have to say though.

My wife's title is "Miss" instead of "Ms". My feeling is that she probably likes it that way anyway.:)

Better change that before a female doctor or PhD comes on and starts giving you the sexist/stereotype rant!
