I reserved a car online using PC code FLT2 for quadruple DL SM per day. Of course, only single miles posted. I filled out the online contact form 2 weeks ago, and got no reply whatsoever. Does anyone have effective corporate contact info for the frequency program? I wouldn't bother but it's 1200 miles I'll miss out on.

I don't have any specific numbers for the frequent flyer help desk, but call the customer care line (1-877-283-0898). Listen to the options, but I believe option 4 and then 0 (or wait through all the options) will get you in touch with a customer care agent who deals with closed contracts (as opposed to currently open contracts--different agents have different access to different sections of the computer system). There may actually be a separate option, though, for frequent flyer mileage questions, though, so listen through them.
