American Express Membership Rewards - Anyone have a copy of the UK Priority Pass application form?

Does anyone have a blank copy of the PP application form for a supplementary Plat. card holder please? If so, if you're able to scan a copy and send it to me as a PDF file I'd appreciate a PM.

I've asked Amex to post one to me, but we're not back in the UK for another 4 or 5 weeks and ideally I'd like to get the application in so the card is there when we return.

Alas I've just chucked mine ! However AMEX are quite happy to fax you out a form and you can fax it to priority pass - assuming you are somewhere with access to a fax of course.

You can fill out an application form online here (http://www.prioritypass.com/Join/How-it-works.cfm). It may be possible to indicate somewhere in the application process that it is for a plat supplementary card holder.
