Depending on when you go, it can sometimes be cheaper to buy a separate ticket to Europe and then an onward to JNB.

Karell travel in Miami is v.g. consolidator for flights ex MIA on SAA. Might be able to help you from IAD as well.

Try 2 tickets. (IAD-LON) & (LON-JNB). You could try to buy the LON-JNB at a London "bucket" shop (check out the Time out magazine or look in the London newspapers for travel ads). & the IAD-LON at a consolidator here. Just some ideas here. Never did it myself yet.
