Singapore Airlines KrisFlyer - No e-tickets if doing EWR-BLR?

A friend was trying to buy a EWR-SIN-BLR RT on Singaporeair.com. He said he was not offered a choice of e-ticket vs paper. The only radio button that it had was paper tickets.
Is that right? Anyway he can get e-tickets? Or is it because India doesn't yet accept e-Tickets?
I thought SQ was pushing towards e-tickets.

Not all destinations yet allow e-tickets. That is outside the control of SQ.

Thanks. In the meantime, i called SQ and they confirmed that select destinations in India, can do e-tickets. They didn't have a list handy. Will post when i get it, if its useful to others.

No eTickets and no internet check-ins: Ahmedabad, Amritsar, Bangalore, Colombo, Hyderabad, Kolkata, and Mumbai.

Ok. He's completed the ticketing. But, he put in his UA MP # for mileage accrual. Now, he can't pull up his booking via Singaporeair.com.
Will this remedy this situation?
Call SQ reservations.
Have them replace the UA MP # with KF #
At checkin, ask them to switch the mileage accrual to UA MP # instead of KF #

In theory, this should work and it does most of the time. In reality, once KF# gets entered into your PNR, there is a possibility that the miles will end up with KF anyway. See this thread, amongst many others:

Well, I advised the friend to call Web Support. He left a voice mail this afternoon that was returned a few minutes ago. Here's what they told him.
1. The ticket needs to be booked when you are logged into your KF account.
2. It doesn't matter what FF # you chose when finishing the booking, if you were not logged into your KF account when making the booking, you cannot retrieve the booking through the Singaporeair.com website.
In this instance, he was trying to book his wife and kid, even though he was not traveling with them. So, when he was logged into his account, it would allow only him to be the "primary" passenger. the second passenger could be either his wife or his kid.
His wife doesn't have a KF #, just a UA MP #. So, eventually we figured out that he could make the booking without logging into the singaporeair.com site. When you do that it allows you to put in pax info for each pax without picking up and defaulting the first pax from your KF account.

One postscript.
Having dealt with UA Reservations, United.com in the past, he was pleasantly surprised when Singaporeair.com website support actually called back to walk him throug his problems. I should say i am impressed too.
