American Express Membership Rewards - Centurion Survey - Unknown Company Name in Address

I received the annual U.S. Centurion survey form today. The address block had my correct name and address, but inexplicably, the company name was not mine or even a company I knew of.

Have any others receiving the questionnaire found it contained the wrong company name in the address block? If so, somebody's database is very messed up!
