Today this came in the mail to two US2s:

"USAirways is committed to providing you with the quality service you expect and deserve. That is why we have created a short online questionnaire for you to offer your opinions about our service and enable us to get to know you better.

Please take advantage of this opportunity to give us your feedback and fill out our questionnaire at http://www.DMFocusOnYou.com

This is your chance to weigh in on service and benefits that matter to you most when you fly. As one of our most valued customers, your participation is particularly important. We truly want to serve you well.

Help USAirways meet your needs and earn 500 bonus miles. Log on to www.DMFocusOnYou.com and participate in our online survey today."

It is a bit longer than most of its kind and hits most of the points that are of interest to us. Have an idea of the amount of travel and which airlines you have used over the last twelve months.

When you get to the end of the survey, they ask you if you would like to get online statements and promotional e-mails (which we already had registered for) and when you agree and give them your e-mail address, they promise you an extra 1000 miles for that. I'll keep my eye on whether they come thru on that for those who had already registered.

Somehow I did not associate that thread with this one because the form of the presentation was different enough.

It appears that they are targeting different headings and language on the mailing to whatever different customer segments that some computer tells them that they have.

If it did not work for someone last month, it might be time to try it again.

Among others, I checked "Other reasons" hoping it would ask me why so I could say "because their web site actually works". That was seriously one of the reasons I switched my flying -- it was too much hassle to call in nearly every time to get something done.
