US Airways Dividend Miles (Pre-FlightFund Merger) - Fly 3 get 1 free bonus q's

Ok, I've booked and flown a bunch of flights since the fly 3 get one fee promotion. Anyone have any idea when/how the "1 free" will be issued? Also, what was the limit on the number of "1 free" you could receive? What is kind of ticket is the "1 free"? Is it like a standard award ticket therefore subject to the same blackout/restrictions?

"About Your Award Certificate: Each award certificate is valid for one roundtrip Coach Class ticket for travel on flights operated by US Airways, US Airways
Express or US Airways Shuttle in the continental U.S. or Canada between September 1, 2004 and June 15, 2005. A Saturday night stay is required. A maximum
of two award certificates may be earned per member. Certificates will be mailed to qualified members beginning July 1, 2004. Awards are subject to certain
