Just got this in an email from Frontier:

I had a pretty quick turnaround on this. I faxed my request on Friday and received an e-mail on Wed. that I had been comped. I'm guessing that's good through 2004, but haven't seen anything definite.

I just emailed them asking for it as well...however since US only uses electronic statements I had to send them an HTML file of my current statement...will they accept that or will I have to print it out and fax it to them?
