American Express Membership Rewards - 1000 MR Bonus for paperless statement question

I received a postcard from Amex today, offering 1000 MR points for agreeing to discontinue my paper statements. I went to the site, http://www.americanexpress.com/paperless , but there was no place to enter the bonus code (4516). Has anyone else seen this one?

Also, the fine print says that you have to stay paperless for a minimum of three calendar months or points will be forfeited.

I got the same plus another card for 1000 MR points if $1,000 in eligible charges (travel-related) is made between 8/1/2001 and 10/31/2001. Bonus ID is 9329.

No website or phone number to call. You need to be enrolled in the Sign & Travel program and charge $1k between 8/1 and 10/31.
