As *G, I get 3 free bags. Can one of them be a bike box, or do I need to pay extra, assuming the box fits the regular size/weight requirements?
If it goes over, is the fee the regular oversize/overweight fee?

Here is what US says about bicycles on the website:

Bicycles

Bicycles will be accepted as checked baggage for a charge of $200 per direction, if over 62 in/157 cm in total dimensions (total dimensions are length + width + height). If the bicycle is under 62 in/157 cm in total dimensions, it will count as a first or second checked bag. One item of bicycle equipment is defined as 1 non-motorized touring or racing bicycle with a single seat.

Bicycles should be prepared for transportation by the passenger. Bicycle must be placed in a card board or hard cased bike container. Bicycles not enclosed will still be accepted, however, the handlebars must be fixed sideways and pedals removed, or bicycle handlebars and pedals must be encased in plastic or foam or similar material for transport. A release form will also need to be signed by the passenger.

If the box is less than 62 inches, it's free. That's unlikely if it's a normal adult bike.

Last time I flew, I was going to bring my bike. The length alone is well over 62 inches...

You're much better off with Fedex

I travel frequently to Rome, Italy. I bought a folding bike and was able to pack it in box exactly 62 inches in length. I did not tell them it was a bike and they accepted it as my regular luggage.

somewhere else on Flyertalk (either Omni or the Fitness forum, I can't remember), someone recommended this place for shipping bikes:

I have a decent bike and I'd never ship it on any airline. Unless moving or doing a race (where you need to get your bike there), I'd just rent a bike.

The shipping might be cheaper here - http://bikeflights.com/ I'm flying up north in a few weeks and shipping my bike via FedEx through them. ~$120 round trip vs $200 each way. :)
