There's also the Ameriprise World Elite card... but this is $50 unless you meet a relatively high spending threshold. If you fly one airline you may want to look into that specific airline's premium card instead (i.e. United Club card, Delta Reserve).

What is covered by that "airline incidentals"? I know that luggage fees are, but not sure if they cover Duty Free purchase?

There are some reports of reimbursements for items listed as non-reimburseable, but none that I recall for duty-free purchases. You can check other threads in the Amex MR forum for details by airline.

I believe Citi Prestige added this feature when they removed Priority Pass (because Priority Pass eliminated access to UA lounges).
