I'll be travelling to Hokkaido this coming Dec, and am looking for scenic onsen whereby the bath is outdoor with snow falling down :D Any recommendation?

If you want one near Sapporo the Hoheikyo Onsen is good.

If you go there I think going mid-December onward will be better as snow in early December may still be light.

+1 for this and it would spoil you as hardly anywhere else could beat it, one of the best in Japan IMHO.
If you are staying in Hakodate region, consider La Vista Hakodate Bay.

Although it is an Onsen on the rooftop of building, it provides superb view overlooking Hakodate Bay and Mt. Hakodate, particularly during sunset.
