Club Carlson - Not able to register

i am new here

i tried http://clubcarlson.com to register

but once i fill the form and click submit button i get error

i tried to email them but when i click submit button ,i get error like

my email submit didn't reach them


2, in the form i found promotion code but i don't have any code or able to find it any code
