The ANA website lists 45 mins - is that really true? I've done international UA to domestic NH several years back and it involved immigration, customs and a terminal change. Obviously domestic to international may not require immigration and customs, but 45 mins is enough for a terminal change?

Are you sure there's a terminal change? I've flown domestic ANA into Narita and out on UA international then into Narita on on UA international and then domestic ANA Narita to Osaka several times at it was always the same terminal.
Last time was JAL domestic to Narita and then out on UA, and that was a terminal change, having to collect bags and get on the bus, then go through exit procedures, etc. 45 minutes is VERY tight for that.
ANA domestic into Narita and then UA international out was extremely easy, since the exiting immigration line is only for passengers on that one plane, not for general passengers using Narita. If bags are checked through, 45 minutes would be plenty of time.

All in the same terminal....45 minutes would seem doable as long as the lines aren't too long at immigration/customs. If you are cutting it close NH will have personnel to help you move things along....

Other than immigration, you also have to figure the time that many domestic flights arriving into NRT have to be bus in (park at a remote gate) espeically if you are arriving on a 320/73G/738. That alone would already be an extra 15min at least.

I wouldn't say 45 min is plenty of time, it depends on time of the day and day of the week. Keep in mind int'l flights board 30-45 min before departure.
