Looking to book first class london to tokyo in the spring, but see both ANA and JAL are coming in twice the price of BA. What am I missing? Would love to fly ANA with the new suite but...

ANA is usually a lot more expensive than BA because a) it is significantly better (IMO) and b) Japanese people are very loyal to Japanese carriers and are prepared to pay extra.

I'm assuming you are looking for non-stop options but EY has a great fare in their suite (http://www.businesstraveller.com/files/News-images/Etihad/Etihad-first-bed-new-1.jpg) (£4700).

Also take a look at CX F to HND. HND will save you a lot of time and hassle and also CX F is one of, if not the best, first products available IMO (£5950).

I see you are also with LH.

Perhaps consider booking with your LH miles or booking business and upgrading with LH miles (I assume this is possible as it is - occasionally - with UA miles).

As for the original question...

My guess is that the overwhelming clientele of NH F fall into one of the following four categories:

1. Senior members of large Japanese corporations whose travel policy allows them to travel F
2. Mostly Japanese (but I presume a few non-Japanese too) people who are overwhelmingly rich enough to pay for F
3. Elite AMC members who upgrade from C
4. The 'occasional guests' (myself included) who are lucky enough to find reward space in F

None of these people are sensitive (in any meaningful way) to the paid F ticket price, for one reason or the other. If they were competing on price, they would have set the price accordingly long ago.
