I have a pre-paid phone service for trips to Italy, using a conventional SIM card with a Siemens phone. This week I'll be getting my iPhone 4S, which has an unlocked GSM slot for a micro-SIM card. If I go to a TIM shop, can I get my service switch to a micro-SIM card and eliminate one phone?

Yes- but I think they will charge you 10 EUR. Be careful also they don't switch you to a more expensive package of services (Iphones are notorious for this in Italy).
