The only international arrivals lounge that I have visited is the LGW lounge and I thought it was quite good.
Is it worth the trek to the FCO arrivals lounge at the Hilton? How does it compare to that of LGW?
If I am renting a hertz car is it another long walk to the rental car from the Hilton arrivals longe?

Location: Hilton Rome Airport, check in at reception
Arriving Envoy Class passengers have access to dayrooms at the Airport Hilton, where they will need to check in at the hotel's main reception desk to present a boarding pass and the invitation received onboard. The rooms provide shower facilities along with breakfast.

At the FCO Hilton, it's just an ordinary room given for a quick shower, shave and change. The breakfast coupon given at reception didn't amount to more than a cuppa and danish, as I recall. I like the LGW arrivals lounge better.
I think you actually walk past the car rental place on the trek to the Hilton, so returning isn't so bad.

It is at most a ten minute walk from the terminal to the Hilton, most of it a covered walk way with a moving sidewalk (if it is working). Hertz is about half way between the terminal and the hotel. (Are you really nuts and going to drive or better yet park in Rome?)
The hotel staff is very pleasant and the rooms are large vs European standards. There were six of us and each could have had our own room, but we chose not to use them all. It was a welcome break to be able to shower, eat and rest up before heading to the train to the termini. Breakfast is supposed to be a croissant and coffee, but they gave us the full buffet free, most likely because it was Christmas morning. The rooms were ours all day if we wanted them.
IMHO it was worth the walk over and back, even with all the luggage. Can't compare to the LGW lounge as the last time I was there US was using the BA lounge.

Is there clear signage to the Hilton after you exit customs?

Yes. the walk way is on the upper level. You will turn left and walk to the end of the terminial. Take the elevator to the upperlevel. The signage is good.
