I received this as well. I already have a DM Visa card, so I don't get why I received this. In addition, I don't get the connection with the shuttle referenced in the e-mail, and the title of the survey mentioning affinity cards. I know, I shouldn't ask such questions!

Thanks for the information. I filled it out and it said 500 miles would be posted in 3 weeks. Let us see if it works.
