US Airways Dividend Miles (Pre-2005 America West merger) - Good Service - Where should I send and to whom?

Was excellent treated today from a rep at the reservation desk for international awards. She called me to inform me about some flight changes for my award flight. And I mentioned that I kept trying to get the departure date changed since weeks. She called me back later to tell me that she found an open seat at another day. I have her name, just need a mailing address for the service center in Winston Salem and a name (head of customer service or similar) to send a quick note.

I once wrote a letter about an agent who was frequently helpful on check-in. She later got a job with the mainline airline in that same city. She originally worked for the affiliated commuter airline.

Hello,

I have sent in several letters on "good service" over the past few years. I always sent them too the Winston-Salem P.O. box and I always got a nice letter back.
