US Airways Dividend Miles (Pre-FlightFund Merger) - What's the procedure for getting comped?

I've got a pal who just moved into PHL and is looking like he's going to fly US for all his domestic travel (Int'l he'll stick to CO for those routes where US does not fly).

He is Gold on CO and I'd like to give him the info needed to be comped to US2 on US.

Without even looking at those threads, what worked for us (my girlfriend and I) when we moved to Charlotte was a fax to the Dividend Miles Service Center at 336-661-8448 explaining what we were after and including copies of our statements from Delta's website showing our status and activity over the past several months.
