I've searched the web and come up with nada.

Do the PR pharmacies dispense antibiotics et al without MD written prescriptions as in Mexico?

Puerto Rico's medical and pharmaceutical profession is much more heavily regulated than Mexico and most likely more regulated than most countries in Latin America;
I am sure you can still convince a pharmacist here and there to give you a course of antibiotics if you are sick but overall I would not count on stocking up on meds while in PR.

think of the rules here the same as US. we have Walgreens, etc.

Same rules as the US, as far as major pharmacies go (Walgreens, CVS, El Amal). However, some smaller pharmacies may sell you just enough to get you by until you go see a doctor. However, that is quite illegal (although seldom enforced).

Guess I won't be stocking up in SJU as I did at the Walmart in PVR.
