US Airways Dividend Miles (Pre-FlightFund Merger) - Remind me about the survey

I know I took a US survey. When I contacted Div. Miles, they called when I wasn't home. Then I got to thinking and did a search. I know I did one survey for a chance to win 500 miles. What was the most recent survey? I want to say it was in July. Thanks.

My 500 miles were FINALLY posted August 23, (after many e-mails to Dividend Miles), but backdated as July 3. Hope this helps.
