US Airways Dividend Miles (Pre-FlightFund Merger) - Exchanging Upgrades for Miles

The information on Preferred Levels indicates that unexpired 800 mile upgrades can be exchanged for 500 mile credits and that the instructions are on the back of the upgrade certificates. Well, I must have OLD certificate stock because I can't see any instructions. Would someone share the procedure and their success for this?
