I have compiled a brief summary of the rules/restrictions and fees for some of the major carriers regarding carryon pets. This thread is intended to be used only as a general pet travel tool and you must contact your airline and confirm their rules/restrictions and fees if you intend to travel with your pet in cabin.
Your pet in-cabin counts as your one piece of carry-on baggage on most airlines.
You must make a reservation for your carryon pet with your airline!
It is highly recommended that you always travel with up to date health documents from your vet for your pet.
If you see any mistakes, wish to add an airline (especially international carriers) or have updated program/fee increases information please send me a PM with the details and the appropriate link for that carrier to refer to and I will update this thread.

The pet kennel is limited to maximum dimensions of 8.5" high x 17" long x 12" wide.
The carrier must be leak proof and ventilated on at least two sides.
There is no weight restriction on pets.
The pet must remain in the carrier and under the seat for the duration of the flight.

ALASKA AIRWAYS – HORIZON AIR
Customers must be 18 years or older to travel with pet in the cabin. A customer traveling with a pet must not occupy an emergency exit row seat. The First Class cabin can accomodate a total of one small pet per flight, and the main cabin can accomodate a total of up to five small pets per flight. A customer may travel with a maximum of two carry-on kennels/containers in the main cabin, only when the adjacent seat is purchased by the same customer.

Due to Hawaii's pet quarantine laws, Alaska Airlines' policy is to not allow pets on flights to/from Hawaii.
Due to an airport remodel project, Horizon Air will not accept pets will not for shipment to or from Reno airport through mid to late 2009. Small pets may still be carried in the cabin.

Virgin America is a pet-friendly airline that accepts cats and dogs in the Main Cabin of the aircraft. Guests with pets cannot be seated in the exit row, first row of Main Cabin or the First Class cabin. Pets checked as baggage are not accepted.
Each animal should be carried in its own kennel (provided by the guest). We allow one pet carrier for each purchased seat. Puppies and kittens must be at least 8 weeks old and weaned. The total weight of the animals and kennel cannot be above 20 pounds.
