The Department of Transportation has established a toll-free number (866-670-3341) for travelers "to share their experiences or insights." Reports can also be filed through the inspector general's Web site (www.oig.dot.gov), the office said in an e-mailed statement

This link (http://www.oig.dot.gov/airtravel04.php) will take you directlyto the form which you can use to submit any kind of feedback in regards to how airlines handled the holiday weekend travel period and how well they accommodated customers needs. The date range that the inspector general is looking for feedback on is only noted as "holiday weekend" so I guess one could take that to mean either the Christmas or New Year's weekend.
FYI also I first read about this online feedback form in
John Mccorkle's email newsletter. The newsletter is very detailed and informative and provides commentary on the state of things at US Airways. The latest issue is a recap of events in 2004, and tells you things about the recent disruptions that management has left out of their press releases. Namely, it is suggested that the recent disruptions are largely due to woeful understaffing and ellimination of certain programs where f/as can earn vacation pay. The newsletter really does a good job of showing you things you would never find in US official press releases. E.g. the number of f/a sick calls are very similar to past years.
