So feel free to get surveyed. It worked for my wife and two sons, but I was told I already took the survey.

This is the same survey that many of us already got. My son got the same one and instead of a drawing he got 500 miles.
I would rather of received the 500 miles instead of a chance to win a trip.

I've already taken the survey (it ells me). I tried to log in under my wifes DM number, but it keeps telling me it's an invalid number.
I get to the survey from the link above. When I go to usair.com, there is no need to "log in" - if I fill in either my or my wifes DM number, it takes me to the account.
So, the above link is the only was I can get there, and it doesn't like her number.
Hmmmmm.

It is 75 questions, but depending on how you answer them. The survey would often skip 5-10 questions, based on the counter on the page, depending on the answer.
