I just received my US3 welcome pack (will it be different than my US2 pack?).

Included was employee praise comment cards, where I could write down the name and employee number and jot down the details.

But why don't they send us complaint cards that list employee names and numbers?

Would anyone be interested in this sort of thing?

Just use the USAirways website to contact Consumer Affairs (http://www.usairways.com/customers/contact/consumeraffairs.htm) for complaints, I see no reason for "complaint cards". Compliment cards are different because it allows you to provide immediate positive feedback to employees (I always just fill out my information then give it to the employee(s) to fill out their information), which with bad situations I usually just want to leave it behind and then report it to the powers that be later. I certainly wouldn't want to be brandishing "complaint cards" around an employee who was causing problems ... it could only make the situation worse.
