ANA Mileage Accrual
Essentially, you earn 1 mile for each Platinum Point that you accrue (see below) + one mile for actual flight miles multiplied by your Elite Bonus level. For more details see below.
http://www.anaskyweb.com/us/e/amc/guide/ft_index.html
As per the ANA USA Platinum Desk on 9/3/04:
Class of Service Bonus:
150%=F, A
125%=C,D,Z, and sometimes J
100%=Y
70%=B, H, K, L, Q, S, W also M’s from USA and G’s from Japan
50%=G’s from USA and M, T from Japan
This thread details mileage accrual from Singapore. It is pretty much in line with above until you get down to the bottom of the barrel stuff. Their may be differences between the orign countries or the information may be slightly off.

Platinum Points... What are they and how are the accrued?
Platinum Points are the points you accumulate which qualify you for elite status. On International flight an AMC member earns 1 PP per flight mile multiplied by the class of service (fare class) multiplier. For domestic flights, the member earns 2 PP per actual flight mile. The Jury is still out on exactly how domestic Super Seat bonus effects PP accrual. Diamond Elite requires 100,000 PP, Platinum Elite requires 50,000 PP and Bronze Elite requires 30,000 PP.
Platinum Point calculation will be based on mileage accrual in relation to the ratio established for fare type used and the basic mileage time used for the sector traveled. Campaign mileage or other bonus mileage does not qualify for point conversion. Points cannot be earned with mileage on flights other than ANA Group flights*2 or the other Star Alliance airlines, or on hotel or card mileage.
Flight mileage from travel on ANA Group flights*2 or the other Star Alliance airlines registered with a partner program does not qualify for point conversion.
Code share flights with applicable airline flight number printed on the ticket operated by airlines other than Star Alliance member airlines are not applicable for Platinum Point accrual.
Code-share flights operated by Fairinc are applicable only when reserved and boarded with an ANA flight number.

As per the ANA Platinum Desk on 9/3/04:
Notes Regarding SuperFlyers Card Eligibility:
This service is not available in the USA so one must apply through the Japanese SuperFlyer’s Desk. While residency in Japan is not required, you clearly need to have bank accounts, an address, and the necessary financial history in Japan that would be required for approval of your credit card application.

Note: Softbank has taken over the wifi concession in NH lounges. It has been a royal pain to get online my last two trips. During my most recent trip in March 2006 the connection was smooth. I already had the encryption settings etc in my PC. There's some bogus deal where you need to activate your day use via your Japanese cell phone, but just tell the desk you don't have one and they'll supply an id and pass. And for gosh sakes, make sure your wifi power switch is "on" unlike the the knucklehead I had to save in the lounge last December.

Minimum check-in/transfer times at NH airports
The way this information reads to me is that you need to allow 3 hours between flights if you are transfering between NRT&HND and approximately 3.5 hours between KIX&ITM. These figures include travel time between airports and the time you need to arrive in advance of your flight. It seems that there is a typo on the KIX page. At the bottom it reads Haneda instead of Itami.
