With YQR and YYT having key-code access MLL's with no dragons, does AC do anything to make sure unauthorized people are not able able to access them? I suppose people could hang out around the doors until they are given an opportunity to sneak in?
No that I would try as I have access, but I was just wondering out of curiosity.

YYT has a keypad for access, and they change the code every day. There usually is someone working (cleaning, tidying and restocking) in the lounge, and occasionally they do check boarding passes.

1-2 Staff usually in YYT, just no front desk so people would be outed easily
YQR No staff, cleaning person every so often but very very small so sneak in at your own risk. But I would suspect some one would out you if you don't belong and they figure it out. No room for freeloaders
I've been in and out of YYT quite a few times over the past few weeks. I've noticed that the staff do a pretty good job of monitoring access particularly when multiple people come in together.
For a while (last year, maybe), a staff person working the lounge would check your boarding pass when you entered and note something on paper.I haven't seen that being done this year, and haven't been checked. I have entered with the code, and I don't know to what extent (if at all) the codes are shared. I have never shared the code with anyone.
The YYT needs to be larger, I think. With the increased (and increasing) traffic here, the lounge is often packed before flights.
As someone who travels to YQR frequently and is just qualifying for E for 2013, how does this work? They give you the access code when you check in on your boarding pass?

Yes at check in they will write it on your boarding pass, if they don't ask

If you've done OLCI and don't feel like checking at the check-in desk downstairs you can go to any AC agent at any gate and get the code as well. I've done that when there has been a line up downstairs.
I'm not in the lounge a lot (I'm only a P) but when I have been in there (maybe four times) nobody's checked my boarding pass. I get the code when I drop off my bag at the check-in counter, or if I forget or have no bag, can get it from a gate agent upstairs as well.
I've got a UA First reservation YQR-DEN-SAN in February; hoping I get access. (There was no business class on YQR-DEN when I booked, but there is now; alas, I've been assigned a seat in E+. Better than plan Y I guess.)

Yes, that's also how it works when connecting at YQR. I've just asked the nearest GA.
