Just got off the phone with CSR at AMEX and insurance for award tickets is indeed not available unless you have enrolled in FATO, that is Frequent Airline Travelers Insurance. It a once a year charge of $8.99 and provides all coverages you would otherwise have.
I don't think I've ever seen an Amex insurance product that's only $8.99/*year* ... Mind telling us more details?
