This wikipost is now very outdated, and if anybody here can edit it, that would be helpful to the people reading it, as they can then get the accurate information. These are all German airports, so opt-outs are permitted. Here's what I can add:

DUS - remove from the scanner-free list: Scanners in most lanes of the B and C security areas, and one scanner in A. To SDOO, just go straight to security area A, and use a WTMD lane, you can access all A, B, and C gates from any security area. More scanners are at some gates, so all US bound pax are scanned, unless they opt-out.

MUC - One scanner in the non-schengen gates area. Presumably for US bound pax.

HAJ - Currently trialing a new generation of MMW scanner, called the QPS200. Not sure of any more details.

FRA - Currently investing in introducing 60 new scanners between May 2015 - early 2016. Some are already in place.

CGN - Scanners planned for introduction in 2016.

Now for a British airport:

LGW - North Terminal: the checkpoint is scanner-free as of now. Scanners could arrive when the new security area is opened in 2016. South Terminal: scanners now used in most lanes.
