I haven't flown OTP-MAD but based on my experience on other routes last year (and talking about Business, of course, as per the title of the thread):
- It's a 4hr flight, so I would assume you get a hot meal
- I'm not 100% sure, but I think that if the plane has IFE (ie: overhead TV) then yes, they will provide headsets
- Eyeshades: no, I don't recall there being any kind of amenity pack (so bring some eyeshades from a previous business class flight, if you want to sleep...)
- Pre-departure drinks: yes
